The artist looks like it’s Frank Cheyne Papé. I’m not sure if there is an official title for the artwork beyond “He usually attended in the form of a monstrous goat.” It’s probably from one of the books he illustrated. Some of these are available for download at Project Gutenberg: http://www.gutenberg.org/ebooks/author/39182.
A good link to the artwork: http://dailywindowonaworldofwonder.blogspot.ca/2012/10/he-usually-attended-in-form-of.html.
